Abstract

Katuk leaf (Saoropus androgynus) is known in South Asia and South East Asia as a traditional medicine to supplement breast milk production. The objective of this study is to assess the efficacy of katuk leaf extract treatment during the study period in the increase of breast milk production for the success of exclusive breastfeeding. As a matter of fact, breastfeeding practice in Indonesia has been decreasing and about 50-80% of pregnant mothers afterwards complain about their insufficient breast milk production in their first week of puerperium. A controlled randomized study was conducted to 80 pregnant women with>=37 weeks gestational age who later delivered at Immanuel Hospital Bandung. The study group obtained the katuk leaf extract, while the control group obtained placebo, daily for 4 weeks. Breastfeeding time was divided into 3 periods: <1 hour, 1-24 hours, and >24 hours postpartum. The participants were observed by phone for 4 weeks. Breastfeeding was considered to be successful if the baby was given only breast milk during the study period without any food and drink supplement. The result showed that in general, katuk leaf extract treatment did not have significant effect on breastfeeding success. Some effect was seen in the earlier breastfeeding: < 1 hour postpartum breastfeeding had more effect to breastfeeding success than 1-24 hours and > 24 hours (p < 0,001). In fact, breastfeeding at < 1 hour postpartum was related with exclusive breastfeeding during the period (p<0,001). Thus, it was the earlier initiation of breastfeeding that had significant effect on breastfeeding success.
