Abstract
The development of information technology which increasingly fast, bring great effect against the development of information services. Distance and time are not obstacles anymore. Any information become faster and easier for being accessed by anyone, anytime, anywhere. Internet as a medium for providing information services was selected because its use is practical and easy for everybody. Through the website media, delivery and information exchange can be done quickly, easily and safely. As the number of people using the internet to access a website increasing, so there’s will be many institutions which make the application on the website, so that all applications can be accessed by people who need them. Application of the computer has been covering almost every sections of the community support in the ease of information delivery. With this application the agent can provide a good and controlled report by manager, wherever and whenever they need it to. By this application the user can get the information about reports and activities which the agent has done. Programming language used in this program is PHP hypertext preprocessor and using a XAMPP database. Based on the results of the design, realization and testing concluded that the application is website based can be a good fit to run the original goal.
